Who would benefit from home health services?
Anyone that has had a decline in health, mobility or is recovering post surgery and has become homebound would benefit from home health services. Home health care is a treatment option available to patients who do not require the full-time care of a hospital or extended care facility, but are under a physician’s care for an acute illness or an exacerbation of a chronic or terminal illness.
Reasons to initiate a home health referral:
  • A recent discharge from a hospital or skilled nursing facility
  • An acute episode from a chronic illness or condition
  • A rehabilitation need due to a decrease in mobility and endurance
  • New medication management and education
  • Disease process education
  • Pain limiting function
  • A recent fall
  • Increased request for assistance
  • Family concerns due to a change in status
  • Wound care management and education
