Home health care is provided by nurses, therapists, home health aides and medical social workers in the comfort and privacy of the patient’s home. The staff at Physicians Home Health Care (PHHC) can treat most illnesses, injuries and medical conditions. The home health team will develop a plan of care based upon clinical evidence of the patient’s individual need for care. We work closely with the family members and the patients in educating them to be proactive in their care, resulting in a faster recovery. Most services are covered by Medicare and are less costly than an extended hospital stay or nursing home.
