There are hundreds of patients who stay home or return home to recover from a medical condition or illness. They want to be as independent as possible, but remain connected to their health care provider to feel confident in the quality and continuity of care after they leave the hospital. PHHC has a comprehensive range of health care professionals, products and services that provide a seamless transition from hospital to home. The home health team will develop a plan of care based upon clinical evidence of the patient’s individual need for care.
